Dirt in ticklish old man's belly on?

I noticed that my grandfather who is very frail has aculated dirt in his belly on. I have tried to scrub it out both while he is awake and asleep and he coils up and is unbearably tickled. How do I remove this stubborn, old dirt from his belly on that is like scabs? Is there a dirt dissolver that is for skin?
